Specialized Claim Forms

In addition to the standard New Patient Intake Form above, please fill out the appropriate form below if your treatments are related to an active workplace or motor vehicle accident claim:

  • WorkSafeBC (WCB) Claims: Complete this form if you were injured on the job and your treatments are being covered through your worker’s compensation board claim.

  • ICBC Claims: Complete this form if you are seeking rehabilitation for injuries sustained in a motor vehicle accident under the Insurance Corporation of British Columbia.

Please Note: Having these forms completed prior to your appointment allows our administrative team to verify your coverage and process your billing seamlessly, ensuring your visit is entirely focused on your recovery.

How to Submit Your Completed Forms

Once you have filled out your paperwork, please submit it to our administrative team using one of the two convenient methods below:

  • Option 1: Submit via Email Save your completed form and email it as an attachment directly to info@mountainviewhealth.ca.

  • Option 2: Submit via Fax If you prefer to print and fax your paperwork, you can send your completed form to our secure clinic fax line at (604) 544-5063.

Note: To ensure a seamless check-in process, please submit your forms at least 24 hours prior to your scheduled appointment so our team can verify your insurance or claim details in advance.
